I. Basis: In accordance with Pingtung County Government Education Department Letter No. 11041514600 dated August 13, 110.

II. Purpose:

(I) In conjunction with the implementation of the 12-year basic education, cultivate students' care for their surrounding environment, embody the spirit of volunteer service learning, and shape a friendly campus atmosphere.(II) Encourage students to assist elderly family members or community members aged 40 and above in their willingness to get vaccinated and make appointments, thereby implementing nationwide epidemic prevention efforts.

III. Project Period: From July 8, 110 to December 31, 110.IV. Incentive Conditions:

Students assist elderly family members or community members aged 40 and above in completing the "Vaccination Willingness" or "Vaccination Appointment" registration on the government's COVID-19 public vaccination appointment platform website: https://1922.gov.tw/vas/.

V. Incentive Method:

For every 2 elderly individuals served, the homeroom teacher or the student affairs office teacher will register 5 bonus points for the student.

VI. Implementation Method:

Students may participate in this project voluntarily based on their time and willingness. They must obtain parental consent beforehand and assist while adhering to epidemic prevention measures. Students who successfully help family members or community elders register their willingness or make vaccination appointments on the government's COVID-19 public vaccination appointment platform must apply to their homeroom teacher or the school with "Registration Serial Number of Successful Registration, Screenshot of Appointment Serial Number." (Application form attached).

VII. This project will be implemented after approval by the authorities, and amendments will be handled similarly.

學生自行斟酌時間和意願自由參與本計畫，並應事先徵詢家長同意，在落實防疫措施下協助，學生須於政府COVID19公費疫苗預約平台協助家中或社區長者登記意願或預約接種疫苗成功者，憑「登記成功之登記序號、預約序號截圖」向導師或學校提出申請。(申請表如附件)

七、本計畫經鈞長核定後實施，修正時亦同。
